EveryKid Sports Pass

You may be eligible for the Every Kid Sports Pass grant. See qualifications listed below:

* Your child is currently enrolled in Medicaid, SNAP or WIC.

* Your child is between the ages of 4-18.

* The sport you are applying for is recreational, not elite or travel.

* The sports season lasts at least 4 weeks.

Grants are disbursed via Chek Virtual Debit Card and can only be used at MYHA. 

If you apply and you are approved, please email us at 4avalanchehockey@gmail.com and let us know your approved amount so we can invoice you.

If this is an emergency, please reach out to a board member immediately. 

Please fill out this form as soon as possible following the incident. Please be as detailed as possible.

This report will be reviewed by the Board of Directors President and the Hockey Development Coordinator / Safety Director. Either of them may reach out to you, and any others who are identified in the report, to gather additional relevant information. They will take the report and all accompanying information back to the rest of the Board to discuss how to follow up. When we have completed the entire process, we will follow up with you to let you know how the situation was handled.

The Monroe Youth Hockey Association (MYHA) was organized to empower and inspire young athletes through hockey, fostering their growth, teamwork, and character.
